Home from hospital service

The home from hospital service is available to vulnerable adults over 18 who live on their own in West Sussex, and in need of some social and practical support after they are discharged from a stay in hospital. The service is supported by volunteers who will visit clients regularly for 6 weeks, this service is free of charge as it is funded by West Sussex County Council.

Our volunteers provide companionship to someone who has little social support,  our volunteers will pop in for tea and  chat or escort someone to a local activity or group if they are able. They can help build confidence with walking and getting about around the shops. Alternatively they can provide practical support to those who are recuperating from their hospital stay by assisting with light shopping, prepare a light snack, picking up prescriptions, accompany clients to follow up GP and hospital appointments.

The service aims to support people to become independent again in their own homes, and the team will signpost and refer clients onto to other agencies to ensure that longer term support can be put in place if needed.

Age UK West Sussex are covering the Chichester and Arun area, also the Mid Sussex and Crawley areas, but we are working in partnership with Guild Care and Age UK Horsham who cover Adur, Worthing and Horsham respectively.
